We partner with Delta Dental of Wisconsin to offer you the choice between two dental plan options — the Basic Plan and Premium Plan. Both plans provide access to a wide network of dental providers and in-network preventive care covered at 100% — no cost to you. The Premium Plan offers a higher calendar year maximum per person as well as orthodontia coverage.

Delta Dental Member Resources

Checkup Plus
Modine participates in Delta Dental’s Checkup Plus program. CheckUp Plus allows you to get diagnostic and preventive dental services without those costs being applied to your individual calendar year maximum. This leaves more flexibility for restorative care that might be needed later.
Evidence-Based Integrated Care Plan

The Evidence-Based Integrated Care Plan (EBICP) provides enhanced dental benefits for those who need them most. This program provides additional cleanings and/or fluoride treatments to individuals with specific medical conditions such as diabetes, pregnancy, specific heart conditions that pose a risk to certain types of infection, kidney failure or dialysis, suppressed immune system, cancer therapy, and periodontal disease.

Amplifon Hearing Health Care

Amplifon’s goal is to help you find the right solution for your hearing health by providing personalized service, high-quality care, and exceptional products.

Accessing your discount is easy:

  1. Call Amplifon at 888.901.0132 and they’ll find a provider near you.
  2. They’ll explain the Amplifon process and help you schedule an appointment.
  3. They’ll send information to you and the provider, ensuring your discount is activated.

Delta Dental Networks

Make the most of your dental coverage by choosing an in-network dentist. Delta Dental has two dental networks to choose from — Delta Dental PPO and Delta Dental Premier.

The Delta Dental PPO network provides the deepest discounts on care and services. The Delta Dental Premier network is a broader network and provides a discount; however, the discount isn’t as large as in the PPO network.
